29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: USCG Base Kodiak, Hangar 1, C130J Fire Suppression Project: on or about 10 October 2019; employees were observed entering an excavation site with unguarded and unprotected measuring approximately 12 feet in depth. The excavation walls were not properly protected for Class "C" soil, and no other form of protective methods exist such as a trench box. This condition exposed employees to hazards associated with cave-ins.

This condition exposed employees to hazards associated with cave-ins.
