29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  a)  At 1018 Snapdragon Lane, Cottages on the Greene, Foley, Alabama:  On or about October 15, 2019 and at times prior; the employer allowed his employees to operate pneumatic nail guns without eye protection while performing house wrapping activities exposing his employees to struck-by hazards.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  a)  At 1018 Snapdragon Lane, Cottages on the Greene, Foley, Alabama:  On or about October 15, 2019 and at times prior; the employer allowed his employees to perform house wrapping activities from job made scaffolds with a work platform that measured only 5 1/2 inches wide and not 18 inches wide as required by the regulations exposing his employees to a fall hazard of up to 13 feet 2 inches to the ground below.
