29 CFR 1926.102(a)(1):   The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a)  Building 7, Colgate Drive, Morristown, NJ.  Employee using or working near a pneumatic coil nailer was exposed to penetrating eye trauma as no eye protection was being used.                                          Violation observed on or about 10/15/2019.

29 CFR 1926.405(g)(2)(iv):   Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a)  Building 7, Colgate Drive, Morristown, NJ.  Employees were exposed to electric shock as the Makita 7-1/4" circular saw was missing its cord protector, also known as strain relief, where the power cord enters the saw.                                          Violation observed on or about 10/15/2019.

29 CFR 1926.451(c)(2)(iv):   Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  a)  Building 7, Colgate Drive, Morristown, NJ.  Employees were exposed to falls of 19 to 20 feet to the ground below from the makeshift "strand board" bucket resting on the forks of the JLG rough terrain forklift as the bucket did not meet the JLG approved fork mounted personnel work platform guidelines for JLG machines.                                          Violation observed on or about 10/15/2019.

29 CFR 1926.451(c)(2)(v):   Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the forks and the fork-lift was moved horizontally while the platform was occupied:  a)  Building 7, Colgate Drive, Morristown, NJ.  Employees were exposed to falls of 19 to 20 feet to the ground below from the makeshift "strand board" bucket resting on the forks of the JLG rough terrain forklift as the bucket was not attached to the forks and the platform was moved horizontally toward the building.                                          Violation observed on or about 10/15/2019.
