Deficiencies noted included, but were not limited to, steel plates that protruded from floor conduit canals and depressions in the concrete left from where it had been damaged, both of which exposed employees to strain/sprain type of injuries.

29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances was not allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made or aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard:   a) Job site, 23 Furnace Street Extension, McKees Rocks, PA: On or about October 15, 2019, the employer did not ensure that floors were maintained in good repair where powered industrial trucks were operated. Steel plates that covered floor canals were found to be damaged and in need of repair/replacement, posing a risk of having the unit's tire(s) get stuck in the canal.

Deficiencies noted during the inspection included, but were not limited to, having a reverse alarm that barely audible above ambient noise levels, operator did not look before backing up, unstable loads were moved and wheel chocks were not used under tractor trailer tires when a forklift entered a trailer for unloading purposes.
