29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On 10/2/2019, employees of D&Z Installs LLC were installing a new roof on Commercial Building at 7400 South Outer Road 364, Dardenne Prairie, Missouri.  Eye protection was not used by employees involved with using pneumatic nail guns during roofing operations.

29 CFR 1926.300(b)(2): Moving Part(s) of equipment exposed to contact by employees or which otherwise created a greater hazard, were not guarded.  On 10/2/2019, employees of D&Z Installs LLC were installing a new roof on Commercial Building at 7400 South Outer Road 364, Dardenne Prairie, Missouri.  The guard protecting the belt and pulleys was removed on a Hitachi EC2610E air compressor exposing employees to caught in-between hazards.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.  On 10/2/2019, employees of D&Z Installs LLC were installing a new roof on Commercial Building at 7400 South Outer Road 364, Dardenne Prairie, Missouri.  Employees were working on an 8:12 pitch roof approximately 30 feet above the lower level without the minimum requirements for a suitable fall protection system.  Employees were not able to recognize fall hazards and the procedures necessary to eliminate these hazards.

29 CFR 1926.501(b)(11):  "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On 10/2/2019, employees of D&Z Installs LLC were installing a new roof on a Commercial Building at 7400 South Outer Road 364, Dardenne Prairie, Missouri.  Employees were installing and removing shingles approximately 30 feet above the ground, without the use of guardrail systems, safety net systems, or personal fall arrest systems.      D&Z Installs LLC was previously citied for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1210836,  citation number 1, item number 1 and was affirmed as a final order on 5/15/2017, with respect to a workplace located at 4720 Haynes Drive, Saint Charles, MO.
