On 10/2/2019, Lifetime Roofing and Renovation LLC operating as the General Contractor for roofing operations at a commercial building at 7400 South Outer Road 364, Dardenne Prairie, Missouri.  The general contractor or controlling employer did not conduct an adequate inspection of the job site, materials, and equipment.  The following hazardous conditions existed:      a)   Sub Contractor employees of D&Z Installs LLC were on the roof of a commercial building              Approximately 30 feet above the lower level without the use of a fall protective system.      b)   Sub Contractor employees were exposed to an unguarded Hitachi EC2610E air compressor,             exposing them to caught in-between hazards.      c)   Sub Contractor employees were using pneumatic nail guns without the use of eye protection.
