29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): On or about October 10, 2019, the employer did not remove from service the Hyster US lift serial number F103548 powered industrial truck that was in need of repair due to a broken propane safety straps which would secure the tank in place. Employees were exposed to a fire and/or explosion hazard if the tank fell off truck. No abatement certification is required for this item.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about October 10, 2019, an employee was operating the JWBS Vertical Band Saw to cut Styrofoam. The employee's left index finger tip was amputated due to improper adjustment of guard. The guard was installed to eliminate an amputation and /or laceration hazard. No documentation is required for this item.
