29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:       (a)J&E Hernandez Construction at 4505 South Cliff Avenue, Sioux Falls, SD 57103: On or about and at times prior to October 15, 2019, the exposing employer did not ensure the employees were protected from injury in that, employees engaged in re-roofing activities on a single-family home were exposed to falls from approximately 15 feet without the use of a fall protection system.  This condition exposed employees to a fall hazard of approximately 15 feet.     J&E Hernandez Construction was previously cited for violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1396121, citation number 1, item number 2 and was affirmed as a final order on July 9, 2019, with respect to a workplace located at 7808 West Stoney Creek Street, Sioux Falls, SD 57106.
