29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a)  Rocky Mountain Mobile Truck Service, Inc. at 10371 E 106th Ave, Brighton, CO 80601: On or before October 16, 2019, the employer did not ensure that each fork lift operator had been trained to operate the forklift. The operator's training did not consist of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace.  This condition exposed employees to the hazard of unsafe forklift operations.

The certification shall include the name of the operator, the date of the training, date of evaluation and the identity of the person(s) performing the training or evaluation.  a)  Rocky Mountain Mobile Truck Service, Inc. at 10371 E 106th Ave, Brighton, CO 80601: On or before October 16, 2019, the employer did not certify that the operators of the forklift had been trained and evaluated as required by 29 CFR 1910.178(l) in the topics as outlined in 29 CFR 1910.178(l)(3). This condition exposed employee's to amputation and struck-by hazards.

A plastic spray container containing diesel was found to be unlabeled and unidentifiable. This condition exposed employees to skin, eye and respiratory hazards.

The cord used to connect the drill press to an electrical source was missing a ground plug.
