29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. a) Loading Dock: On or about October 8, 2019 and at times prior thereto; the loading dock lift does not sit flush to the concrete floor, and metal mail carts catch on the uneven edges as dock expediters and contractors unload truck trailers, creating slip/trip/fall hazards and struck-by hazards. b) Loading Dock: On or about October 8, 2019 and at times prior thereto; the dock plate has jagged metal edges, exposing dock expediters and contractors to lacerations, cuts, and abrasions. c) Loading Dock: On or about October 8, 2019 and at times prior thereto; dock expediters and contractors are exposed to caught-in hazards where the dock plate used to load truck trailers creates an uneven working surface and an opening large enough for a foot to fall into. Note* Abatement certification IS required for this item.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment. a) Loading Dock: On or about October 8, 2019 and at times prior thereto; the dock plate was modified to sit flush against truck trailer edges, but in doing so, created jagged metal edges, exposing dock expediters and contractors to lacerations, cuts, and abrasions during loading and unloading operations when dock plate must be lifted by hand, or by using a chain that is attached to the loading dock platform. Note* Abatement certification IS required for this item.
