29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) Sebastian Martinez at 10260 Washington Street in Thornton, CO 80229: On or before 10/09/19, the employer did not ensure that each employee was protected from the hazard of projectiles to the eyes when using or working near pneumatic nail guns. The employees were performing roofing work without the use of safety glasses on a two story residential roof. This condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501: (a) Sebastian Martinez at 10260 Washington Street in Thornton, CO 80229: On or before 10/09/19, the employer did not ensure that employees were protected from fall hazards while roofing a residential apartment complex. The employees were performing roofing work without the use of fall protection on a roof pitch of up to 8/12. This condition exposed employees to fall hazards of approximately 20 feet.
