29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:  a)  Roof Evolution LLC at 10260 Washington Street in Thornton, CO 80229 : On or before 10/09/19, the employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person.  Subcontractor workers were shingling a two-story residential apartment complex without the use of fall protection.  This condition exposed workers to fall hazards of up to approximately 20 feet.   OSHA defines a "competent person" as someone capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to workers, and who has authorization to take prompt corrective measures to eliminate them.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501:  a)  Roof Evolution LLC at 10260 Washington Street in Thornton, CO 80229:  On or before 10/09/19, the employer did not ensure that subcontractor workers were protected from fall hazards while roofing a two-story residential apartment complex.  Subcontractor workers were shingling a roof with up to a 8/12 pitch without the use of fall protection.  This condition exposed subcontractor workers to fall hazards of up to approximately 20 feet.
