29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On or about October 15, 2019; front of residential site; employees working on the ground level near the scaffold system were not wearing hard hats to protect them from falling objects such as but not limited to tools or materials used in the masonry process. 29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) On or about October 15, 2019; front of residential site; the working platform of the east-side scaffold had 2 planks missing, exposing the employees to an estimated 13-foot fall hazard while conducting masonry work.    b) On or about October 15, 2019; front of residential site; the working platform of the north-side scaffold had 1 plank missing, exposing the employees to an estimated 17-foot fall hazard while conducting masonry work.  c) On or about October 15, 2019; front of residential site; the working platform of the west-side scaffold had 1 plank missing, exposing the employees to an estimated 17-foot fall hazard while conducting masonry work.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   a) On or about October 15, 2019; front area of the residential building; the employees were observed using the scaffold system frame and cross-braces to access the working platforms of the scaffold system exposing the employees to a fall hazard of approximately 13-feet to 17-feet.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
