29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part: On or about October 11, 2019, at a worksite located at 570 East Cuyahoga Falls Ave., Akron, OH 44310, during commercial roofing work, the employer did not have a safety and health program implemented at the worksite. The employer did not ensure employees were provided safety and health training or that there was a system in place to recognize and correct unsafe working conditions.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about October 11, 2019, at a worksite located at 570 East Cuyahoga Falls Ave., Akron, OH 44310, during commercial roofing work, employees were exposed to a falls of approximately 25 feet. The employees were performing roof installation activities on a commercial building low slope roof with no fall protection system/device. Bella Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1417810, citation 01, item number 001 and was affirmed as a final order on September, 25, 2019, with respect to a workplace located at 800 West Waterloo, Akron, OH 44314. Bella Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1083574, citation number 01 item number 002 and was affirmed as a final order on October 22, 2015, with respect to a workplace located at 1447 Martin, Magadore, OH 44260.
