29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.     On or about October 16, 2019, employees engaged in framing/roofing tasks were working from an aerial lift without wearing a body belt and lanyard attached to the boom or basket.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     On or about October 16, 2019, employees engaged in framing/roofing tasks while on the walking/working surface (trusses) were not protected from falls.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.      On or about October 16, 2019, at the previously addressed job site, the portable ladder(s) used to access the working surface (trusses) did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
