Where mechanical handling equipment is used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways were not appropriately marked.    a.  On or about October 16, 2019, in the warehouse, mechanical handling equipment was used on a regular basis to move materials.  Aisles and passageways were not in good repair or kept clear.  The aisles and passageways were blocked or were too narrow to allow safe passage with mechanical handling equipment, thereby exposing employees to struck by hazards from falling materials.     b.  On or about October 16, 2019, in the warehouse, mechanical handling equipment was used on a regular basis to move materials.  Aisles and passageways were not permanent or marked to indicate flow of traffic or movement.
