On or about October 16, 2019, at the above addressed jobsite, an employee working alongside another employee cutting metal window trim with a circular saw did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated    a.  On or about October 16, 2019, at the above addressed jobsite, employees installing window frames from a Genie S-65 aerial lift, approximately twenty-eight (28) feet above the ground, were not wearing personal fall arrest systems, thereby exposing the employees to fall hazards.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.     a.  On or about October 16, 2019, at the above addressed jobsite, employees installing window frames from a Genie S-65 aerial lift, were not trained to recognize the hazards associated with working from an aerial lift.  The employees were thereby exposed to the hazards associated with falls.
