The employees work where flexible cords are used as substitutes for fixed wiring. On 10/16/2019 employees were exposed to hazardous conditions where a flexible cord was used to power a stationary piece of equipment exposing the employees to electrical hazards.

The employees work with pieces of equipment powered by damaged flexible cords. On 10/16/2019 employees were exposed to hazardous electrical conditions where a damaged flexible power cord with exposed internal wiring was used to supply power to a portable vertical panel saw.

The employees work where the directions of travel to the nearest exits are not easily apparent. The employees are permitted to work in area where there are no markings of the permanent aisles to prevent struck-by by hazards where forklifts are used. On 10/16/2019 employees were exposed to struck-by hazards from forklifts which operated where there were permanent aisles which were not marked.
