Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling.  a) 4200 Summit Bridge Road, Machine Shop, Middletown, DE, On or about October 21, 2019, the employer failed to ensure employees did not use relocatable power taps (RPTs) that were permanently secured to tables and to provide power to portable heating mats/pads, exposing employees to electrical hazards.  For Reference:  Underwriters Laboratories (UL), refers to power strips as Relocatable Power Taps (RPTs) and, in its "General Information for Electrical Equipment Directory" (sometimes called the UL white book or UL Directory) contains instructions that require UL-listed RPTs to be directly connected to a permanently installed branch circuit receptacle; Relocatable power taps are not intended to be permanently secured to building structures, tables, work benches or similar structures, nor are they intended to be used as a substitute for fixed wiring.
