29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided.    Location: Sidewalk to the top of sidewalk shed    a)  On or about 10/17/2019, there was a break in elevation  of 27 inches from an extension ladder to the sidewalk shed.  Employees used the ladder to access the shed platform.

29 CFR 1926.1053(b)(1): The ladder extension was positioned such that deflection under a load, by itself, would cause the ladder to slip off its support.    Location: Top of sidewalk shed to the working platform of the set-back area     a) On or about 10/17/2019, employees used an extension ladder to access the working platform on the set-back of the building. The ladder slid left while the employee was climbing up the ladder.
