29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide. On the west side of the construction site, employees engaged in cornice activities on a residential home from a single pole scaffold worked from a platform that was not 18 inches wide. The employees were exposed to fall hazards of up to 15 feet to the lower level.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On the west side of the construction site, employer did not provide a scaffold ladder or a ladder that could extend up to the walking platform where employees could access and egress the pole scaffold safely. The employees were exposed to potential fall hazards up to 15 feet to the ground below.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On the west side of the construction site, employees were working from the pole scaffold without fall protection system while installing cornice work on the exterior of the residential home. The employees were exposed to fall hazards approximately 15 feet to the ground below.
