On or about October 16, 2019 at Diamond Plastics Corporation, Plaquemine, LA location an extension cord was used as permanent wiring for the line 2 Co Pilot 256 printer. Employees use the Co Pilot 256 printer to change over the piping every 7 - 10 days.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about October 16, 2019 at Diamond Plastics Plaquemine location employees used the Beller machines on line 3 and line 5 without adequate strain relief for electrical wiring. Instance a) On the line 3 pump vacuum tank electrical wiring was exposed the strain relief was not adequate. Instance b) On the line 5 Beller table the emergency stop did not have adequate strain relief.
