On October 14, 2019, an employee received burn injuries when flammable FVP Brake Clean vapors ignited and exploded inside of a hydraulic tank that was being cleaned and modified.

29 CFR 1910.1200(g)(1): Employers did not have a safety data sheet (SDS) in the workplace for each hazardous chemical which they used: 35 Birch St, Milford, MA: On October 16, 2019 and at times prior, the employer did not have safety data sheets available to employees for the hazardous products they used. On October 14, 2019, an employee received burn injuries when flammable FVP Brake Clean vapors ignited and exploded inside of a hydraulic tank that was being cleaned and modified. The SDS was not available and the SDS warned that this product was flammable and was to be used only outdoors or in a well ventilated area.

29 CFR 1910.1200(h)(2): The employer did not provide employees with effective information on hazardous chemicals in their work area at the time of their initial assignment and whenever a new chemical hazard the employees have not previously been trained about was introduced into their work area. Employees were not informed of the following: the requirements of this section; any operations in their work area where hazardous chemicals are present; and, the location and availability of the written hazard communication program, including the required list(s) of hazardous chemicals, and safety data sheets as required by this section. 35 Birch St, Milford, MA: On or about October 16, 2019, the employer did not train employees on the hazardous materials they used and did not make safety data sheets available. The employer did not have a written hazard communication program in place to be trained to employees.
