29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing residential roofing operations.  This was most recently observed at 17609 O St, Millard, Nebraska on October 18, 2019 when 4 workers were on a residential structure with an eave height over 11 ft. and a pitch of 12 in 12 and they were not using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards while using ladders.  This was most recently observed at 17609 O St, Millard, Nebraska on October 18, 2019 when 4 workers were observed using a ladder to gain access to the roof of a residential structure and the ladder did not extend at least 3 feet past the upper landing surface.  The roof eave height was over 11 feet above the ground.
