29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and de-energization of the machine or equipment had been accomplished: a) On or about October 17, 2019, in the Maya Structure-I at Shell Deer Park Refinery, the employer failed to ensure the 200# steam system was isolated and blinded-off before they began work on the Motor Operator Valves (MOV).
