29 CFR 1910.25(b)(3): Stairs did not have uniform riser heights and tread depths between the landings:  a) Yard - On or about October 18, 2019 and at times prior to, the employer did not ensure that the height of the risers was uniform on the stairway leading to the West Salem Machinery Co. Grinder operator station, thus exposing employees to fall hazards of up to about 3 feet, 6 inches.

29 CFR 1910.28(b)(11)(ii): 29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails per Table D-2 of this standard; stair width less than 44 inches (1.1 m), and two open sides required to have one stair rail system on each open side:  a) Yard - On or about October 18, 2019 and at times prior to, the employer did not ensure that the flight of five (5) risers on the stairway leading to the West Salem Machinery Co. Grinder operator station was equipped with a standard stair railing that included an intermediate rail on the open sides, thus exposing employees to fall hazards of up to about 3 feet, 6 inches.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above the floor or platform were not enclosed:  a) Yard - On or about September 28, 2019 and at times prior to, the employer did not ensure that the sprocket and chain assembly on the West Salem Machinery Co. Grinder was operating with the guard in place, thus exposing employees to caught-in hazards.
