29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: A) 194 South Wood Avenue, ISELIN, NJ- Sixth Floor: On or about October 11, 2019 the employer failed to ensure that two floor openings on the fifth floor were fully covered with covers and covers were secured to prevent accidental displacement, exposing employees to fall hazards.
