29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates or markings were maintained in a legible condition: Employees were exposed to struck-by hazards. Employees engaged in material handing activities at an active construction site using the TH360B Caterpillar forklift 5CR4188, serial number SLEO4807, were exposed to stuck-by hazards in that the data plate, that identifies the various rated capacities of the forklift at different load configurations, was not maintained in a legible condition. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

(Construction Reference: 1926.602(d)): Employees were exposed to struck-by hazards. Employee engaged in material handing activities at an active construction site using the TH360B Caterpillar forklift 5CR4188, serial number SLEO4807, was exposed to stuck-by hazards in that comprehensive forklift training was not provided prior to permitting forklift operation. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
