29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about October 18, 2019, at the address of 1311 River Road in Maumee, OH: the employer did not assure that employees were provided with safe access to the upper landing surface of a two story structure by extending the ladder side rails at least three feet above the surface to be accessed. Employees were exposed to a 25 foot fall hazard.

It had been evaluated during employee and employer interviews that employees had been trained, though the employer could not produce a written certification of their training.
