Observed on or about 10/11/19 The employer name was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1412823, citation number 1, item number 2, and was affirmed as a final order on 8/22/2019, with respect to a workplace located at 469 Harrison Street, Emmaus, PA 18049.

The portable extension ladder used for access/egress to a roof of a two story residential home did not extend at least 3 feet above the upper landing surface. Observed on or about 10/11/19. The employer name was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1412823, citation number 1, item number 3, and was affirmed as a final order on 8/22/2019, with respect to a workplace located at 469 Harrison Street, Emmaus, PA 18049.
