29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    Fabrication Shop:     a. Storage Rack Area: The exit door leading to the outside was difficult to open and did not open fully due to a pile of dirt outside in front of the door.    b. Welding Area: The exit door opened to a deteriorated and uneven step that could create a tripping hazard.    c. Welding Area: The path to the exit door was partially obstructed due to the pipe racking reducing width of egress path to approximately 17 inches.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    Fabrication Shop: A medical evaluation program was not implemented for employees who voluntarily wore half mask respirators during welding and cutting operations.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  Fabrication Shop: Hazard Communication information and training was not provided to employees that work with propane that powers the forklifts and metals that are welding and cut, generating metal fumes in the work place.
