29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) On or about October 18, 2019 and at times prior thereto employees were exposed to a 23 foot fall hazard to the ground while standing on a concrete conveyor belt, where adequate fall protection was not used. b) On or about October 18, 2019 and at times prior thereto employees were exposed to a 21 foot fall hazard to the ground while standing on and climbing guard rails and an I-beam, where adequate fall protection was not used. c) On or about October 18, 2019 and at times prior thereto employees were exposed to a 6 foot fall hazard to the metal platform on three sides of the conveyor belt, while standing on the conveyor belt, where adequate fall protection was not used. NOTE-Abatement certification and documentation are required for this item.

NOTE-Abatement certification and documentation are required for this item.
