29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized:  On or about October 15, 2019, an employee was exposed to noise levels at 154% of the allowable 8-hour time weighted average (TWA) sound level (90 dBA) while dismantling pallets at the pallet dismantler and using a Sawzall.  This level exceeds a noise dose of 100%.  The equivalent dBA level of 154% is approximately 93.1 dBA. This level exceeds an 8-hour TWA of 90 dBA. The sampling was performed for 390-minutes during one shift on October 15, 2019.
