29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).    On or about October 21, 2019, employees were exposed to a cave-in while performing waterline repair at a residential home located at 3085 East Ridgewood, Seven Hills, Ohio 44131.  The employees were working in a trench that was approximately 7 feet deep and the employer did not ensure that they were using adequate cave-in protection.
