29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a.    On or about October 18, 2019, in the body shop and spray paint booth, the employer had not developed a respiratory protection program for two painting employees who were required to wear tight-fitting respirators while pouring, mixing, and spraying paints, hardening agents, reducers, sealers, and solvents that contained chemicals including, but not limited to: xylene, ethylbenzene, butanone, toluene, methanol, acetone, heptane, naphtha, methylcyclohexane, hexamethylene diisocyanate, and butyl acetate.

On or about October 18, 2019, on the back wall at repair bay 2, the eyewash station available to employees who handle corrosive chemicals had not been serviced and was unsanitary.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a.    On or about October 18, 2019, between repair bay 5 and repair bay 6, the right grinding wheel's tool rest on the Dewalt D758 Bench Grinder was adjusted to greater than 1/8-inch from the grinding wheel surface.  b.    On or about October 18, 2019, between repair bay 5 and repair bay 6, the Dewalt D758 Bench Grinder had no tool rest installed for the left grinding wheel.

29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4)(a):    a.    On or about October 18, 2019, an employee operating an unguarded Dewalt 4 1/2-inch DWE small angle grinder to cut off a vehicle muffler pipe was exposed to laceration and puncture wound injuries from the cut-off blade.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  a.    On or about October 18, 2019, in repair bay 6, the 115-Volt power cord for the fixed wall fan motor had broken insulation at the plug with bare conductors, exposing employees to an electric shock hazard.  b.    On or about October 18, 2019, in repair bay 6, the 220-Volt power cord for the vehicle lift had broken insulation at the plug, exposing employees to an electric shock hazard.  c.    On or about October 18, 2019, in repair bay 2, the 220-Volt plug had been altered in a method not approved by the manufacturer to wire a second power cord from one plug to power both automotive lifts from a single outlet receptacle, exposing employees to a fire and shock hazard.  d.    On or about October 18, 2019, in repair bay 1, a 115-Volt portable trouble light was plugged in without a bulb in the socket, exposing employees to an electric shock hazard from the empty socket.  e.    On or about October 18, 2019, in repair bay 1, on the wall, the 4-receptacle 115-Volt outlet box had arcing damage to one of the 4 receptacles, exposing employees to fire or electric shock hazards.  f.    On or about October 18, 2019, in repair bay 1, a 115-Volt portable trouble light that was used as an extension cord to power the clock had an open light bulb socket, exposing employees to an electric shock hazard.

On or about October 18, 2019, in the alcove next to repair bay 1, the junction box was not covered, exposing employees to 115-Volt shock hazards.  c.    On or about October 18, 2019, at the mezzanine staircase, the junction box was not covered, exposing employees to a 115-V electric shock hazard.  d.    On or about October 18, 2019, on the mezzanine, the 4-receptacle outlet box was missing a gland on the cable exiting the upper punch-out hole, exposing employees to a 115-Volt electric shock hazard.
