Hecla Drive, Louisville, CO 80027: On or proceeding October 21, 2019, the employer did not provide a program for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person designated by the employer.  Subcontractor employees were removing shingles without the use of fall protection.  This condition exposed these subcontractor employees to fall hazards of approximately 30-35 feet.
