Many of the base frame legs did not bear on screwjacks, base plates or mudsills; instead resting directly on the ground. The section in front of the entry door rest upon a blue base frame assembly that did not have cross-bracing on one side of the base legs and was out-of-plumb. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) Employee(s) installed stucco to the front exterior of a single-family house at 281 Fleet Avenue in South Plainfield NJ, working atop a 3rd tier scaffold platform.  No safe means of access was provided such as ladder or stair tower; necessitating that employee(s) climb the frame ends and/or braces to ascend/descend the scaffold, which exposed them to the hazard of falling up to 18 feet.
