29 CFR 1926.451(e)(2)(iv): Hook-on and attachable ladders shall be specifically designed for use with the type of scaffold used: a) Job site, 2984 Sidney Street, Pittsburgh, PA: On or about October 21, 2019, during construction activities, the employer did not ensure that employees had a safe means of accessing the scaffold located inside the concrete wall forms in that employees would climb a ladder on the exterior of the wall forms, then climb several feet of existing rebar to get up and over the wall forms and down onto the scaffolding located inside.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use: a) Job site, 2984 Sidney Street, Pittsburgh, PA: On or about October 21, 2019, during construction activities, the employer did not ensure that ladders were properly inspected in that a ladder used to gain access to/egress from the core interior and/or the scaffolding inside the core had faded, marred or otherwise illegible labeling.
