Location: a) By the doorway on the northwest side of the jobsite: On or about 10/21/2019; Employees were spraying water on ACM material as it was being loaded into containers. They sprayed from under a wall that had been identified as damaged and weak. The employer had not provided instruction to the employees on the weakened condition of the wall and the need to remain away from the area.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognize hazards that were causing or likely to cause death or serious physical harm to employees exposed to crush by and/or struck by hazard. Location: a) By the doorway on the northwest side of the jobsite: On or about October 21, 2019; Employees were spraying on ACM material as it was being loaded into containers. They sprayed from under a wall that had been identified by an engineer prior to the start of work as damaged, weak and unstable. The structure had visible cracks. Employees worked on the site using heavy machinery to remove the debris from the damaged structure.
