29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a ) Jobsite: Carriage House Apartment, 1515 Penn Ave, Wilkinsburg PA , On or about October 18, 2019 - Employees were not using all the parts of a personal fall arrest systems and was not attached to the lift while working inside the JLG aerial lift at heights six feet or more above a lower level.
