29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: USCG Base Kodiak, Hangar 1, communications and avionics room: on or about 11 October 2019; an employee was observed during the inspection working without a protective helmet and did not have immediate access to one. This condition exposed the employee to hazards associated with falling or flying objects.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: USCG Base Kodiak, Hangar 1, avionics room: on or about 11 October 2019; employees were exposed to four energized 120 volt electrical power circuits while painting adjacent walls. This condition exposed employees to hazards associated with electrical shock and burn hazards.

29 CFR 1926.96: The employer did not ensure employees wore proper footwear which meets the requirements and specifications in American National Standard for Men's Safety-Toe Footwear, Z41.1-1967. USCG Base Kodiak, Hangar 1, communications and avionics room: on or about 11 October 2019; during the inspection an employee was observed not wearing proper foot protection while hanging ceiling tiles and mounting office doors. This condition exposed the employee to hazards associated with falling objects.
