29 CFR 1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2-1969: a) 13 East Main Street, Fort Johnson, NY - On or about October 16, 2019, employees were using an aerial lift as crane, attaching rigging gear to the bottom of the basket to lift and set trusses. This violates the aerial lift manufacturer guidelines and thus exposed employees to a fall and crushing hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) 13 East Main Street, Fort Johnson, NY - On or about October 16, 2019, employees were exposed to a fall hazard while setting trusses on a commercial building. Employees were not utilizing fall protection and were exposed to a fall hazard of 12 feet.
