29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. The employer failed to ensure that the eyes of employees are protected from flying particles while working on a residential roof. This most recently occurred on or about October 23, 2019 on the jobsite located at 1854 88th Street, Omaha, Nebraska 68124. Employees were observed using hand-held pneumatic nail guns to perform roofing operations on a residential home without any form of eye protection exposing them to eye injuries.

This was most recently documented at 1854 88th Street, Omaha, Nebraska 68124 on October 23, 2019 when employees were observed conducting roofing operations on a residential structure while working from heights greater than 8 and 10 feet without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. The employer failed to protect employees from fall hazards when conducting roofing operations on residential structures. This was most recently documented at 1854 88th Street, Omaha, Nebraska 68124 on October 23, 2019 when the employer allowed employees to use a 24 foot aluminum extension ladder that did not extend at least 3 feet past the roof upper landing surface being accessed.
