29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval:  Beal AFB, KC 135 Warehouse: The employer modified the GRADALL rough terrain fork lift by adding an aftermarket and oversized fabricated metal personnel platform. The oversized platform extended more than 3 feet beyond the forklift wheel base from both side and it was not attached to the forks or the mast arm. This modification was performed without the manufacturer's written approval. The forklift could overturn exposing the employee to crushing hazards.

Specifically, ANSI B56.1-1969, Section 604 Travelling G.3. which states "on all grades the load and load engaging means were not tilted back if applicable, and were not raised only as far as necessary to clear the road surface".  Beal AFB, KC 135 Warehouse: An employee was observed driving a GRADALL rough terrain forklift with the load engaging means elevated above the driver cabins. The forklift was carrying an over sized fabricated metal personnel platform (16 ft. 3 in long X 3 ft. 3 in w X 4 ft. H). The personnel platform was not attached to the forks or the mast arm. Employees were exposed to crushing hazards
