The employees performing this work were atop a scaffolding pick platform without the benefit of any type of fall protection.

Sunwest Development on or about October 23, 2019 at the address of 1810 East Perry Street, Port Clinton, OH: The employer did not ensure that the employees were using an extension ladder that extended at least three feet above the landing. Employees were exposed to falls while using the step ladder to access the working level of the structure.
