Bob Miller & Sons, LLC, 5100 Buffalo Road, Erie, PA 16510, on or about October 22, 2019 and times prior thereto: An employee was walking/working on the truss work of the structure and was exposed to a fall hazard of 16.6-feet. The employee was not utilizing acceptable means or methods of fall protection including, but not limited to, guardrail systems, safety net systems, and/or personal fall arrest systems.

The ladder was not fully extended and was only 2-feet above the surface of the roof. The ladder was not otherwise secured and/or provided with grasp devices.
