29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    A.)  Bob Miller & Sons, LLC, 5100 Buffalo Road, Erie, PA  16510, on or about October 22, 2019 and times prior thereto:  An employee was walking/working on the truss work of the structure and was exposed to a fall hazard of 16.6-feet.  The employee was not utilizing acceptable means or methods of fall protection including, but not limited to, guardrail systems, safety net systems, and/or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    A.)  Bob Miller & Sons, LLC, 5100 Buffalo Road, Erie, PA  16510, on or about October 22, 2019 and times prior thereto:  An employee was utilizing a metal ladder to access the roof of the structure.  The ladder was not fully extended and was only 2-feet above the surface of the roof.  The ladder was not otherwise secured and/or provided with grasp devices.

Bob Miller & Sons, LLC, 5100 Buffalo Road, Erie, PA  16510, on or about October 22, 2019 and times prior thereto:  An employee was walking/working on the truss work of the structure without fall protection, and was exposed to a fall hazard of 16.6-feet.  The employer did not have a written certification of the employee's fall protection training.
