29 CFR 1904.32(b)(2): The Summary of Work-Related Injuries and Illnesses (OSHA Form 300A or equivalent) was incomplete:  The employer failed to complete the OSHA Form 300A or its equivalent for the las three calendar years, including 2016, 2017, and 2018. The employer failed to include the street address, city, state, zip, annual average number of employees, total hours worked by all employees and the certifying official signature, title and date.

29 CFR 1904.40(a): At this establishment, the OSHA 300 logs were not available for review upon request by an authorized government representative. Under Part 1904, you must provide copies of the records within four (4) business hours.   The employer failed to provide OSHA 300 logs and the summaries to the representative of the Department of Labor, within the four hour time frame.  The copies of the OSHA 300 log and its summary for the last three years, 2018, 2017 and 2016 were not provided in a timely manner.
