29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)  The employer is failing to provide training on the chemical hazards association with painting.  This violation was most recently documented at a worksite located at or near 3220 Farnam St., Omaha, NE 68128 on October 23, 2019. The employer has an employee painting light poles without having provide the employee any training on the use of the chemicals on site or the safety data sheets that cover those chemicals.

This was most recently documented at 3220 Farnam St., Omaha, NE 68128 on October 23, 2019. A 16 foot portable extension ladder being used by an employee to tape off  a light post was using the rung to support the ladder instead of the two side rails.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov:      The employer is failing to ensure that employees are protected from the hazard of lack of injury and illness identification. This violation was most recently documented at a worksite located at or near 3220 Farnam St., Omaha, NE 68128 on or about October 23, 2019.  The employer did not notify the OSHA Omaha Area office of an employee hospitalization that took place on October 18, 2019 within 24 hours of its occurrence.  The Area Office was notified of this incident on October 23, 2019. An employee was hospitalized when they were using a 16-foot extension ladder leaning on a light post and the light post base collapsed, causing a head injury.
