OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from being caught between the cage and the rigging during a tandem lift using a hoist: a) On 10/7/2019, three employees were in the process of installing a pulverizer cage (8000 lbs.) onto a pulverizer shaft. The employees were using two Harrington Hoist (rated at 6000 lbs. each) and two Atownley single leg chain sling (rated at 8800 lbs. each) to place the cage onto the shaft. During this process the load shifted, an employee attempted to grab the cage resulting the left hand being caught between the cage and the rigging.

29 CFR 1910.184(c)(6): Safe operating practices. Whenever any sling is used, the following practices shall be observed: Slings shall be securely attached to their loads. a) On 10/7/2019 the employer did not ensure that the two Atownley single leg chain slings (rated at 8800 lbs. each) were properly secure to move a Pulverizer Cage (8000 lbs.). During the process of moving the cage, the slings slipped and caused the load to shift. Employee #1 reached to steady the load catching the left hand between the cage and a sling.
