On or about October 21, 2019, employees were exposed to an 11 foot 3 inch fall hazard, in that, they climbed down a 20-foot extension ladder while holding tools and materials in their hand(s).

29 CFR 1910.23(c)(7)(i): The employer did not ensure that ladders placed in locations such as passageways, doorways, or driveways where they can be displaced by other activities or traffic were secured to prevent accidental displacement:  a.  On or about October 21, 2019, employees were exposed to an 11 foot 3 inch fall hazard, in that, the extension ladder used by employees to access the roof was set up with its feet in a parking space near the side entrance of the building without protection from accident displacement.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling:   a.  On or about October 21, 2019, employees engaged in performing roof patch work on a 3/12 pitch roof of a commercial building were not protected from falling 11 feet 3 inches to the ground below.

On or about October 21, 2019, there was not a written Hazard Communication program requiring training and safety data sheets to be available for employees who utilized APOC plastic cement to repair roof leaks, exposing employees to skin and eye irritation.
